Dear Officer

Re: Julian McDonald, Age: 68 years

I am writing to transfer Mr McDonald, who is recovering from a total left knee joint replacement surgery, for immediate post-op care and rehabilitation.

Mr. McDonald has been diagnosed case of gout and osteoarthritis for the past 8 and 10 years, respectively. He is a smoker, alcoholic, hypertensive, hypercholesteraemic,  obese, and has PTSD. He lives alone in a caravan and is allergic to penicillin.

The surgery on Mr McDonald was performed under general anaesthesia. Significant post-op pain required 48 hrs ketamine infusion.  Keflex was given in addition to normal 24-hour post-op antibiotics because his CSU grew Staph Saprophyticus. During his hospital stay, he was provided a nicotine replacement.

Today, Mr. McDonald is slow to mobilise partly from pain, is agitated, and is insomniac. He is advised to have formal sleep studies for his somnolence and snoring.
Other than painkillers, his discharge medications include Zyloric 300mg daily, Lipitor 20 mg at night, Kanvina 300mg daily, Nicabate 21mg patch,  Targin 20/10 twice daily, and Oxycodone 5-10mg 6 hourly. Removal of sutures is planned for 10 days later and a review appointment with the specialist is planned for 7/9/18.

Your help regarding his physiotherapy, occupational therapy, and assessing the suitability of his return to caravan, would be greatly appreciated.
