Dear Dr Hinds

Re: Yuxiang Meng , Age:21 years

Thank you for seeing this patient, an overseas student chef from China, who has presented with features suggestive of an acute manic episode. Your management of his current condition would be greatly appreciated.


Mr Meng lives under the care of his uncle and speaks very basic English. On 2/3/11., while being treated for URTI, he was observed to be elevated in mood, tangential and delusional. A referral was made to the local community mental health for the urgent assessment of his condition and diazepam 10mg four times a day was prescribed to be given under supervision.


On his next visit, his symptoms were improved and his CT scan and blood tests were normal. A provisional diagnosis of non-organic mania was made and he has commenced on quetiapine 50mg twice daily. Also, the dose of diazepam was reduced to 10mg twice daily or three times a day.


Today, the patient is accompanied by his uncle who is reporting some abnormal behavior including discarding his medications. On examination, he is elevated in mood, pressured in speech, and loose in associations. He is refusing any medication.


I would appreciate it if you could assess and manage his condition. Once he is stabilized, he can be referred to a Chinese-speaking psychiatrist.
