Re: Mr Brett Collister, DOB: 20 November 1970


I am writing to refer Mr Collister, a factory foreman, with blood tests consistent with type 2 diabetes mellitus for further evaluation. 


Mr Collister is married with three children. He has no significant past medical history except from being over-weight, with a BMI of around 30. He also received treatment for shoulder and knee pain last year, which were mainly pain killers and physiotherapy.


Mr Collister presented to the clinic on 4 January 2015 with tiredness and sore eyes. In addition, he also felt dizzy. His blood tests for cholesterol and blood sugars were requested as his symptoms were persistent for 3-4 weeks. 


On today’s visit to the practice, Mr Collister is still feeling tired with a decrease in vision. His vitals are grossly normal. However, his blood sugar levels, both fasting and random, are found to be elevated on review of his blood investigations. His HbA1c level is 8.5% with an increase in cholesterol levels. 


In view of the above, he is likely having type 2 Diabetes mellitus and needs further assessment and management by you.
