I am writing to refer Mr
Smith, who has been recuperating from a cerebrovascular accident. He is being
discharged today and requires ongoing care and management from your facility.
Mr Smith was admitted on
30th September with the complaints of left sided weakness associated with
headache, vomiting, dizziness and mild confusion. During his stay in hospital,
his vital parameters were monitored every fourth hourly along with speech and
physiotherapy. Also, he was commenced on intravenous fluids and a light diet
was suggested for him. However, he has made a remarkable improvement in his
general condition, his speech and left sided weakness has to be improved.
Regarding his social
background, Mr Smith is a wharf labourer and lives with his wife in his own
house.
Upon discharge, it would
be greatly appreciated if you could arrange an appointment with dentist,
podiatrist, audiologist, dietitian and occupational therapist. Please continue
his speech and physiotherapy accordingly. Kindly arrange a social worker
assistance for him.

And lastly you
can write social history after 1st
paragraph also.
Kindly recheck
this in case notes also, you have used the verb “to be” in it (I guess in case
notes it is “has been” improved) [speech and left sided weakness has to be improved].
