Dear Dr White,


Re: Mr George Poulos


I am writing to refer Mr. Polous a 45 years old man, who has signs and symptoms suggestive of disc prolapse with radiculopathy for further evaluation.

Polous is married with three children. He is a known smoker ad drinks alcohol frequently. He has a sedentary life with a BMI of 27. Please note that Polous is allergic to Pethitide, penicillin, and radiographic contrast agent.

On 21/06/14, Poluos developed lower back pain after lifting heavyweight. When he first paid a visit to my clinic, the pain was localized in the lower back which was severe in nature. His rest of examination was unremarkable except musculoskeletal. Regrettably, he had lost his lumbar lordosis along with a limited extension of the trunk. He has been experiencing pain while flexing his body. However, sensory and motor reflexes of the lower limb were unremarkable. He has advised analgesics as well as bed rest with a follow-up of 1 week.

On 28/06/14, He has been reviewed with pain radiating to the right lower limb. Despite analgesia flexion of the trunk was limited to the mid-thigh. He had been advised to be reviewed after 1 week again. 

Today Polous has lost lumbar flexion. Similarly, the rest of the movements are restricted. Examination revealed loss of light touch sensation over the lateral distal calf and plantar aspect of the foot. Additionally, the right ankle reflex was mute.

In light of the above, I would be grateful if you could evaluate Polous condition immediately.
