Dear Doctor,
Re: Mr.Darren Walker.
I am writing this letter to refer you Mr. Walker, a 40-year-old married man, who has signs and symptoms suggestive of prostatic cancer for urgent evaluation.
Mr. Walker is married with two sons. He smokes 20 cigarettes a day and drinks 2-3 beers a week. Please note that Mr. Walker's mother is hypertensive and his father is a known case of prostatic carcinoma.
Initially, on 09/07/12, he presented to my clinic. His blood pressure was 165/90 mmHg and his pulse was regular with a rate of 80 per minute. His weight was 85 kg at that time. He was advised to reduce his weight and stop smoking. Moreover, a prostate-specific antigen test was requested. There were no other remarkable Sindings.
When Mr. Walker came for the next visit yesterday, he had reduced smoking and lost 8 kg of weight. His blood pressure was 146/80 mmHg. However, a digital rectal examination revealed an enlarged prostate and the PSA reading was 10.
In view of the above signs and symptoms, I believe he needs further investigations including a prostate biopsy. I would appreciate your urgent attention to his condition.
