Dear Dr Carlson,
Re: Margaret Leon DOB 01/08/1952
I am writing to refer a 60-years-old male, who has signs of colonic carcinoma for further assessment and colonoscopy.
Mrs. Leon does not smoke or drink. Her father died of bowel cancer at the age of 50. Please note that She had undergone a colonoscopy 3 years back which was normal.
On 14/01/2012 she first presented at my clinic with borderline hypertension and raised body mass index. Mrs Leon was advised to reduce weight and do regular exercise. A review appointment was scheduled after 3 months.
On 25/04/12, she presented at my practice again. Her vitals were normal. She lost 8 kilograms during the last 3 months.
On today's review, she was anxious when she noticed blood after the passage of stool twice. She had lost 11 kilograms over the last 3 months. Her BP was 180/95 mmHg, while the rest of the examination was unremarkable. She has been prescribed Alepam 15mg 1 tablet before bed for a short time to relieve anxiety.
In view of the above, a suspicion of bowel cancer is raised. Therefore, I am referring this patient to your care for a colonoscopy and further assessment.
