Dear Doctor,
RE: Mr. Dave Cochrane; aged 55 years
I am writing to refer Mr. Dave, who presented today with symptoms related to severe shortness of breath and chest pain. His further management including admission to cardiology unit for stabilization if required would be greatly valued.
Today, he had complaints of chest pain, difficulty in breathing and seemed anxious. Examination revealed bilateral ankle edema and high jugular venous pressure. His apex beat was located in 6 intercostal space and crepitations were found in both lungs along with noticeable dyspnea.
Regarding his past medical history, he had been diagnosed as a case of left ventricular failure and was commenced on furosemide 40 mg per day and digoxin 0.25 mg per day. He had also taken broad spectrum antibiotics due to features of chest infection seen on chest x-ray. His condition improved after he reduced smoking and continued taking his medications.
Please note that Mr. Dave was a regular smoker and used to drink alcohol previously. Moreover, he was not found exercising regularly.
I would highly appreciate your urgent assessment of Mr. Dave .
