Candidate: Good Afternoon

Examiner: Good Afternoon

Candidate: Nice to meet you again. How are you so far?

Examiner: I am fine doctor.

Candidate: Well, based on your symptoms and after doing fasting blood sugar twice this month, I believe that you have what is called in medical language Diabetes mellitus, have you ever heard of that?

Examiner: I have heard of diabetes. Is it the same as sugar diabetes.

Candidate: Let me explain this to you. If you any question or concerns you can interrupt me during my conversation. Diabetes is basically increased sugar in blood. It is not caused by eating too much sugar. It is caused mainly by absence or insufficient production of insulin, or an inability of the body to properly use insulin.

Examiner: But doctor my question is I don't have eaten any sugar and on two blood tests you are saying me that I am having sugar Diabetes.

Candidate: I really appreciate your concern. As I have told you it is not caused by eating sugary food. It is basically a caused by insulin shortage or ineffective insulin receptors due to overweight. Moreover, World Health Organization has set some protocols after a number researches that blood sugar more than 126mg/dl on two different occasions in fasting condition confirm the diagnosis of diabetes.

I see that must be a very difficult situation for you but the good news is that there are several options available which will help you. Would you like to hear more about that?

Examiner: Yes doctor.

Candidate: First of all, I would like to tell you something about the initial management of diabetes. we will start with low dose Metformin. All you have to do is to restrict yourself from eating too much carbohydrates. Now if prompt initial measures are taken that will help you in in prevention of some serious complications of kidneys, eyes, heart, brain and feet. Am I clear so far?

Examiner: Doctor it is too early to start treatment of diabetes.

Candidate: No XYZ. Actually there are some serious complications of diabetes. If prompt initial measures are taken that can help prevent some serious complications of kidneys, eyes, heart, brain and feet.

Examiner: I want more tests to be done please.

Candidate: XYZ I understand your worries. We will definitely run some tests. you have to do your ECHO, renal function tests, fundoscopy, EMG studies.

Examiner: Ok doctor.

Candidate: However. I would recommend some life style medications like daily exercise, swimming, eating low Carbohydrates food, having sound sleep, and checking fasting blood sugar at home at regular intervals. Please note that if you find increase in fasting blood sugar, you have to come back for checkup. I would recommend a follow-up visit after one month.

Examiner: Ok doctor thank you.
