Dear Doctor,
Re: Andy William
I am referring this patient to you, who has a long-term
history of obesity, for further evaluation and management.
Andy William is 65 years old and has a weight of 155.5kg.
His BMI is approximately 56.6kg/m2 despite participating in weight loss
programmes and consulting dieticians.
William is hypertensive and is a type II diabetic. The patient
is suffering from gout as well. His
HbA1c is 6.1%, with morning blood glucose levels between 100 to 130
and triglyceride levels of 201mg/dl. His serum insulin is 19 IU/ml. For the
management of diabetes, he is taking 30 units NPH before breakfast 70 units
NPH before and after dinner and metformin 850mg. His other medications include Atorvastatin
10mg, Lisinopril, Nifedipine and Allopurinol.
The patient has been obese since childhood and takes three meals
each day, dinner being the largest of them.
Moreover, he visits restaurants weekly. The patient's partner is
encouraging and is concerned about his health. He wants to learn more about the
surgical options available for the treatment of obesity.
In view of the above, kindly recommend suitable treatment
options including bariatric surgery if required.
