Dear Dr. Townsend,
Re: Emma Johnson; aged11years


Thank you for seeing my patient, an 11–year–old girl for the management of viral induced asthma.

She presented today with complaints of fever, runny nose and inflamed throat.  Chest auscultation revealed wheeze on both sides. Her PEFR is 140L/min.

Emma Johnson first presented on 30.11.09 with the same complaints. She was prescribed Paracetamol and monitoring of PEFR.

On subsequent visits her fever and rhinorrhea were resolved, but respiratory symptoms including night cough persisted. She was prescribed Ventolin, Sulbutamol and Corticosteroids, to which she responded well.

Emma Johnson's family history is strongly suggestive of Asthma, as her mother and 15 years old sister are asthmatic. She does not have any other allergy history.

In view of the above I am referring her to you for further evaluation and management.
