Dear Dr. DiCoccio, 

Re: Bethany Tailor, 35 years of age 

Your patient, Ms. Tailor, admitted herself on 1 March 2018 with decompensated schizophrenia. She is now ready for discharge and follow-up at your clinic.

On admission, she was experiencing significant thought disorders, including thought blocking and latency. She was also exhibiting delusions and experiencing auditory command and visual hallucinations.

During her stay in the hospital, Ms Taylor was placed back on her medications, her mental condition was stabilized and she can focus on her activities of daily living. Her insight is now good and judgment fair. Her nursing management in the hospital focused on compliance with her antipsychotic medications, behavioural control, and therapy. Since 10 March, she has not reported visual or auditory hallucinations.

Ms Tailor is on oral Risperidone 4mg nightly. Additional oral risperidone 1mg can be administered as needed twice daily for agitation or psychosis. She will be discharged from the hospital to her apartment where she lives alone. She will follow up with you to continue her treatment of chronic schizophrenia and to avoid non-compliance with her medications or substance abuse.
