Dear Dr. Bradbury

Re: Ms. Isabel Garcia


I am writing to refer back Ms. Garcia, who is being discharged today after being treated for bacterial meningitis, for the follow-up treatment and management of any complication.


Ms. Garcia, on her initial presentation, had a headache, photophobia, and pain in joints. She was afebrile but her neck stiffness test was positive. She had a petechial rash over her legs and abdomen and a bruise over he r left arm. Her blood CRP level was elevated and her LP result showed the picture of  typical bacterial meningitis. She was advised ceftriaxone 2g I/V in b.d, dexamethasone 10mg before the first dose of the antibiotic, and then 10mg I/V every 6 hourly for 4 days. Antibiotic was changed to benzylpenicillin after the culture report revealed the causative agent to be Neisseria Meningitidis.


Ms. Garcia is responding well to the treatment. Her family members are counseled regarding the immunization and the department of Human Services was notified of the disease.


I would appreciate it if you could also counsel the close contacts of the patient regarding initial signs and symptoms of this disease and chemoprophylaxis if required.
