Re: Mrs. Ann Howard, aged 36.

Dear Dr. Jiminez


I am writing to refer Mrs Howard to you for assessment and management of her intestinal problem. 

Mrs Howard is married with 3 children. She had a spontaneous abortion two weeks back. Please note that she also has a history of ovarian cystectomy and appendicectomy. 

On 20/01/2007, Mrs Howard presented to the clinic with the complaint of sudden pain in the left lower quadrant of the abdomen. Her vitals were grossly normal. However, her tummy was tender on the left lower quadrant with a palpable mass. 

Mrs Howard reported a slight improvement in pain on 21/01/2007. She had a bowel movement after 3 days, which consisted of hard stool, stained with blood. 

Today, Mrs. Howard visited the practice with a tense abdomen. Her pain is getting worse after eating. Her haemoglobin levels are 9.3 g/dl, and she has no bowel movements including flatus. I suspect a surgical underlying cause. 

In view of the above, I am referring her to you for further evaluation and management of her condition. If you have any questions, please contact me.
