Dear Doctor,

Re: Mr. Tom Cribb

DOB:23/05/1982


I am writing this letter to refer you Mr. Tom Cribb, a 28-year-old labourer who has signs and symptoms suggestive of renal mass for further evaluation.

Mr. Cribb is married with no children. He smokes 5 to 6 cigarettes a day and 2 to 5 units of alcohol a week. Please note that Mr.Cribb's father is hypertensive and his mother had breast cancer.

Initially, he presented to my clinic on 12/05/10, with a complaint of very severe pain in the right lower abdomen which radiated to the groin. The pain was associated with nausea but no other remarkable findings. Therefore, a diagnosis of ureteric colic due to renal stone was made. He was prescribed Diclofenac sodium for 5 days. Furthermore, he was advised to drink more fluids with regular exercise. An IVP and UFR report was requested for the next visit.

When Mr. Cribb came for the next visit, the IVP report revealed an enlarged right kidney. Therefore, an abdominal ultrasound was requested to check the kidney condition.

Today, Mr. Cribb presented with mild abdominal pain which was relieved with Panadol. The ultrasound reports revealed a mass in the right lower abdomen with severe hydronephrosis of the right kidney.

In view of the above, I believe he needs further investigations including a CT scan. I would appreciate your attention to his condition.
